docs: update Cloudflare Pages example to use wrangler-action#136

cloudflare/pages-action is deprecated; Cloudflare now recommends cloudflare/wrangler-action@v3 with `pages deploy` command syntax. Also fix actions/checkout version (v6 → v4).
